Community Health Assessment  ADPH Survey

The Alabama Department of Public Health's (ADPH) goal is to serve the citizens of Alabama with the highest quality of service possible. As part of this goal, we need your assistance with implementing the 2019 Community Health Issues Survey.

This survey is part of the Community Health Assessment  ADPH utilizes to identify health issues and challenges within Alabama's communities. Knowing what communities consider to be the most important issues will help ADPH better serve all communities in the future. The survey is for those older than 18 years of age and should take less than 10 minutes to complete.

Below are the short links for the survey.
