Library Forms

The following forms are available in pdf format and can be copied and faxed to 727-9282. They will be routed to the correct library department. Click on the corresponding link to access each specific form. Forms will only be accepted for Tuskegee University affiliated persons. This service is not available to persons not affiliated with Tuskegee University.  (Note:  Online forms will soon be available)

Under certain conditions specified by law, libraries and archives are authorized to furnish a photocopy or reproduction. The Library Computer Lab contains 30 Dell All-in-One PCs. It normally cannot be reserved because it is a general lab for student use.  But if the B.I. Lab is not available or not large enough to accommodate the class, then the Library Computer Lab can be reserved, occasionally. Reservations should be made at least a week in advance.
